Amie’s priority is your health and continued wellness. She’s in it for the long haul, but she can’t do it for you. A better life is waiting for you, one filled with energy and new adventures, but it’s up to YOU to make it happen!

Congratulations! You’re on your way to feeling better, living stronger and longer, and looking vibrant! For over 19 years, Amie has helped people reclaim their health. Every client is unique and so is Amie’s approach to their health. Each client receives an individualized wellness program designed to meet their needs and accomplish their goals.

Step 1 Make an appointment:

Step 2​ ​Complete the required forms:

These are found under the forms tab and are labeled Comprehensive Health History and Personal Intake Form. The forms must be completed and submitted through this website. Please, do not print and return at your first visit. These forms must be received at least 3 days prior to your initial appointment with Amie. The forms are as follows:

  1. A health history

  1. Symptom assessment

  1. Client agreement

  1. 3-day diet record

Step 3​​ Submit Additional Information:

If available, please send by email to amiehornaman@gmail.comany recent blood test results or other diagnostic test results.

Step 4​​ Meet Amie:

This is your scheduled initial consultation. The appointment should last 60-90 minutes. Amie will review with you the information from your submitted questionnaires. She will also ask for additional information, if needed. Together with Amie, you will identify your wellness goals and Amie will recommend dietary and lifestyle changes.

Step 5​​ Follow Up Appointments:

This 30-45 minute appointment will occur every 2-4 weeks. The purpose of these appointments is to review your progress, discuss any challenges, and develop appropriate responses.

While in person meetings are available, most clients find telephone conferences are more convenient and beneficial for these follow up appointments.
